His true form emerged once Leon blew up a canister near him. His body split open, his upper torso detaching from its legs while being held up by a large Plagas spinal cord. He grew two huge arms with pinchers out of his back, all in an attempt to kill Leon.

Leon manages to hurt the creature so much that the lower part gets detached; Mendez becomes an upper torso that swings around the barn with his new arms. Extremely deadly and faster then ever, this form gives Leon all he can handle but Leon succeeds in the end.
